Waiver: Registrants and their accompanying family members, friends, and pets participating in the Fairfax Pets on Wheels, Inc. “Paws for a Cause” 3K Walk to be held in Fairfax, Virginia on Sunday, October 2, 2011 hereby assume full responsibility for the risk of bodily injury, death or property damage, and hereby release, waive, discharge and covenant not to sue the City of Fairfax, Weber’s Pet Supermarket, Fairfax Pets on Wheels, Inc. or their respective successors, assignees, officers, employees and agents for any and all loss or damage therefore on account of injury to person or property, whether caused by negligence, breach of contract otherwise arising out of or resulting from any act of omission, while traveling to or participating in the 2011 Paws for a Cause 3K Walk. The undersigned acknowledges that by executing this reservation form he/she grants the Fairfax Pets on Wheels, Inc. authority to use any and all photographs of the undersigned for all lawful purposes to promote the Fairfax Pets on Wheels, Inc. in any future communications, both public and private.
